On the other hand, when i receive a text from a short code that asks, "Please complete your patient form in advance of your appointment.

I googled the URL and didn't see anything that shouted "scam" and did see something that suggested eye care. Ahah, i do have an appointment in a couple of weeks. I wrote and inquired if it was theirs. They replied, "Yes we currently are working in providing this as a better service. We appreciate your feedback and confirm the text message is from our practice."
OK, fine.
I go to the site and read and confirm receipt of privacy policy. I have to say, it pretty much says they share it with whatever makes their lives (business) easier. Like with this SMS message thing, hrmph.
Then i get to the next form:
Um, no.
I remember being cranky about this organization when we first moved here because they blithely sent automated text messages about my glasses being ready to our land line. This seems like more blind IT usage.
So, the form says
teamvision.us redirects to https://home.teamvision.us/auth/login which mentions Amway in the footer?! Also "Copyright © 2023 TeamVision by Signature Equipovision, LLC." which mentions Amway. Righto, not trusting an Amway affiliate with my medical info.
